Community Based Care Management

East End United Community Center has partnered with Fayette County Behavioral Health Administration to help connect our community with resources that can help them with their basic needs. 

 

The Community Based Care Management Program (CBCM)  is intended to enrich lives by providing choice and access to resources that encourage hope, independence and recovery. CBCM provides a holistic approach to overall mental and physical wellness while encouraging collaboration among community-based organizations such as the East End United Community Center.
